Biometric Screening Information for All Active, Non-Medicare Retired and COBRA Participants and Spouses Eligible with the California Ironworkers Field Welfare Plan
IMPORTANT NOTICE OF CHANGES TO YOUR BENEFITS
Currently, you are being sent a package of material notifying you that the date to get your Biometric Screening has been extended to October 31, 2013. Any changes as a result of not getting your Biometric Screening have been delayed until January 1, 2014. However, it is important that you review all of the contents of this mailing so you can be prepared to make a choice to participate in the Reinforcing Smart Choices Promiseand retain your current coverage under the California Ironworkers Field Welfare Plan. Failure to comply with all of the requirements for the Reinforcing Smart Choices Program will result in your placement into the new Basic Plan of benefits effective January 1, 2014.
The following documents are included in the mailing:
Cover Letter (includes your unique identification number) – The cover letter includes your unique identification number that identifies you in our system and eliminates the everyday use of your social security number for identification purposes. This number is being provided to you so you will be able to access and use the Quest Diagnostics Online Scheduler to make an appointment for your biometric screening. Keep this letter for safekeeping so you can readily access your identification number if needed. In the case of those members on the Active Fee-For-Service Plan, this is the same number that is currently listed on your identification cards.
Reinforcing Smart Choices Promise Guide (English and Spanish) – The Promise Guide introduces the Reinforcing Smart Choices Promise and outlines all of the necessary steps you need to take in order to retain your current benefit level. Failure to get a biometric screening and to sign the Promise by the deadline will result in you being placed in the Basic Plan with reduced benefits effective January 1, 2014.
Reinforcing Smart Choices Promise Election Form (English and Spanish) – You must sign and return the Election Form and get your biometric screening by the October 31, 2013 deadline in order to retain your current level of benefits. You must do both; get your screening and sign the Election Form. Under this program, if you and your spouse (where applicable) fulfill the Promise Commitments by October 31, 2013, you and your family will stay in your current health plan. On January 1, 2014 your current health plan will be renamed the Premier Plan. If you, or your covered spouse, do not fulfill the Promise Commitments by October 31, 2013, you will be moved into a Basic Plan on January 1, 2014.
Kaiser Biometric Screening Confirmation Form (where applicable) - This form is only for the use of those members who participate in Kaiser. Instructions are on the form and the completed form needs to be returned to Kaiser at the address noted on the bottom of the form. This is an important step since Kaiser will use this “confirmation” form to verify that you have completed your biometric screening and qualify to retain your current level of benefits.
Kaiser List of “Kaiser-On-The-Job” Centers – This is a list of Kaiser facilities that our Kaiser members can call and make their appointments to complete their biometric screenings. Please be sure to take both your Kaiser Referral Form and your Kaiser Confirmation Form with you to your appointment.
PLEASE NOTE: If you are a Kaiser participant, you are receiving two sets of forms, one for the member and one for your spouse. Each of you must fill out the forms separately in order to comply with the program requirements. If you are not a Kaiser participant, you can ignore these forms.
California Field Ironworkers Trust Funds Change of Address Form – It is important that you keep your current contact information up to date with the Trust Fund Office. In the event you change your address and don’t notify the Trust Office, your mail will be returned (including any checks – both vacation and pension) and held until you provide your current contact information. A bad address could result in you not receiving important notices about your health plan, etc. This form is being provided to you for future use in the event you move.
Summary of Material Modifications(specific to your Plan) – A Summary of Material Modification (SMM) was prepared for your specific plan. The SMM will advise you of specific changes that will be made to your Plan effective January 1, 2014. The information is very important to you and your dependents. While your dependents are not required to get a biometric screening, any choice you make will affect both you and all of your eligible dependents.
Please take the time to review all of the materials when they arrive in the mail and call
the Trust Fund Office at 1.800.527.4613 if you have any questions regarding any of the
enclosures in the mailing. If you do not receive the mailing by August 15, 2013, be sure to notify the Trust Fund Office so we can mail you another package of material.
