Please don't forget to notify your Local Union of ANY change of address. Notifying the Trust Fund Office does not include any notification to your union office. You must provide that information to your local union.

By completing this form you may be entitled to an extension on your welfare coverage and/or additional pension credits. This form must be completed if you are intending to withdraw your Annuity Balance due to Total and Permanent Disability.
